Disk not formatted. Broken disk?

Not looking good, I'm afraid. The magnetic fields on a floppy surface slowly degrade over time. A lot of floppies, particularly the cheap ones, only had a 10 year life span unless you specifically went and refreshed them. You could try contacting a data recovery center to see if any specialized equipment exists that can recover something from the weak fields that remain.
